Requirements

STEP 1: Get Registered

Register online or download our "How To Begin" registration packet and fax to 321-406-0610

Registration can be completed over the phone. All phone payments will require a registration signature form - sent via fax. (321-406-0610)

A complete registration packet is located at the back of our policies and procedures manual. Curriculum will be sent when book deposits or full payment has been made.

STEP 2: Send Documentation

Before a student is officially registered, all required documentation, and a signed and notarized contract agreement must be received at the registration office.

Registration does NOT begin until all documents have been received in or office. Payment is also due, unless financial arrangements have been made with the director. (See Curriculum and program choices)

	Previous school information
	Copy of student's legal birth certificate
	Student immunization and health records
	Copy of an IEP from public or private school - if your child has received the evaluation
	Copy of report card - identifying passing to the next grade
	Copy of transcripts - for 9th grade and above
	Signed and Notarized Contract Agreement
	Signed Acknowledgement of State Requirements
	Two passport or formal pictures 1” x 1” – for Student ID
